We created ViaQuest Hospice to make a difference – a difference in every life touched by serious illness. We’ll bring a very special kind of care into your life and the lives of those you love.
ViaQuest Hospice offers these services:
Together with you, your family and your physician, we’ll create a plan of care completely tailored for your lifestyle, goals and wishes. You or your loved one no longer have to worry about going to the hospital and the high risk of infections and other illnesses.
With any symptom change or shortness of breath, our nurses will come to your home or facility to care for you. Our team includes:
Our staff is available on-call 24/7 to provide comprehensive solutions and dedicated support systems available to you in your home.
We also don’t want you to worry about costs for care, medications or medical equipment. Our services are covered 100% by Medicare, Medicaid and most private insurances. So, you get everything you need in relation to your illness, including medications and medical equipment.
One of the most significant benefits of our care is its support for the family caregiver. We bring in an extra layer of care, much-needed stress relief, and practical support that shares in the caregiving. We also help family caregivers feel in control by providing education on the disease and ensuring they know what to expect and have someone to talk to about what worries them. We’re also here for emotional support and assistance in gaining any needed resources.
Our patients often tell us they wish they had started hospice care sooner. Our support is most effective when we can help early on; this helps ensure you gain the full benefit of our care and support. Many physicians find that the ViaQuest Hospice program greatly enhances and extends the care they provide, and our care extends the lives of their patients.
Give us a call at 855-289-1722 and we’ll help determine if our hospice services are right for you. This program is currently available in Ohio, Pennsylvania and Indiana.
