We offer comprehensive, community-based therapeutic services to children, adolescents and adults with mental health and behavioral needs. All mental health and behavioral services are billed directly through the client’s insurance; we never use waiver dollars for services provided. Treatment needs may involve:
Health Services Coordination (HSC) is a team of nurses who provide coordination of medical services to adults with Developmental Disabilities in Franklin County. HSC does not include Nurse Delegation of Services. This is a Franklin County funded program.

What is Health Services Coordination?
It is a Franklin County funded service that provides coordination, communication and medical management for individuals with intellectual disabilities.
Is Health Services Coordination similar to a Home Health Agency?
No and in fact HSC Agencies are prohibited from providing any skilled care to our disabled individuals. We do, however, work very closely with Home Health Agencies to ensure that individuals’ medical needs are met. When needed, we will link our individuals with an available Home Health Agency to provide on-going skilled medical care.
How often do you visit the homes?
We do at least one home visit every three months however additional home visits may be needed depending on the individual’s medical needs and the level of training the DSP’s have.
What is the benefit to using HSC Services?
How is Quality Assurance associated with the HSC agencies?
As required by the state, we need to perform a quality assurance site review once every three years. The QA’s are associated with the site not the individual. There may or may not be an additional QA done when an individual moves to a different site. We work with our Providers to ensure that they are aware of the information that is needed and help them to make sure that each site is working according to state rule.
How do I enroll our an individual for HSC Services?
Unlike HPC services where it is individual driven, HSC Services are Provider driven. Therefore, if you are an agency Provider, you must use the same HSC Agency to provide HSC services to all of your individual’s. Exceptions can be made with approval from the County Board. If an individual moves from one HPC Provider to another, their HSC Agency may also change. For an individual to receive HSC Services, the individual must meet the criteria based on the self-med assessment and it must be written in their Ohio ISP. If an individual does not meet the criteria, and the team believes that HSC services are needed, exceptions can be made by the SSA. An example of this would be an individual coming out of a hospital for a medical event and needs extra oversight until they have recovered.
